Eligibility
- Entry age is 18 to 70 years. Dependent children may be covered from 5 months
- family coverage includes the insured, spouse and dependent children aged 5 months to 25 years. Sum insured is based on monthly income from gainful employment as prescribed by the insurer.
Passing a listed condition does not mean the bank will approve an application.

- Proposal details: name, address, age, occupation, sum insured and income information.
- Claim form for every claim.
- Death claims: death certificate; post-mortem certificate where conducted; , police investigation, viscera/FSL, legal-heir and succession certificates where required.
- Disability claims: government-doctor certificate confirming disability percentage and employer leave certificate where applicable.
- Travel or modification benefits: original expense proof, doctor certificate, workshop/civil-engineer estimate and completion receipts where applicable.
- Optional medical expense claims: discharge summary, medical reports, original invoices/bills and payment receipts.
- Hospital cash or home convalescence: discharge summary, treating-doctor recommendation and attendant payment receipt.
- The insurer may request additional documents and identity/address proof.
